NHIA Releases Draft Practice Standard for Pharmacist Documentation

NHIA is seeking public feedback on the draft Infusion Practice Standard: Pharmacist Documentation – Initial Assessment and Plan of Care. This standard was developed in response to a growing need for clear, consistent guidance on pharmacist documentation practices within the home and alternate-site.
The draft provides structured recommendations on developing and reviewing plans of care, assessing patient and clinical information, reconciling medications, documenting interventions, and ensuring accurate and complete pharmacist assessments.
NHIA is requesting that public stakeholders review and provide feedback on the draft practice standard. Comments on proposed practice standards play a key role in the standards development process. The public comment period opens Wednesday, November 19, 2025 and closes Monday, December 8, 2025.

NHIA Comments on Proposed EIP LCD
September 8, 2025
NHIA submitted comments to the DME MAC medical directors regarding their proposed local coverage determination (LCD) for external infusion pumps (EIPs). DL33794 would expand Medicare Part B coverage for blinatumomab (Blimcyto®, Amgen) for certain indications.
